1. SERVICES, TELEHEALTH & MEDICAL PROVIDER OVERSIGHT
Laboratory Testing Services
Synergistic Labs LLC facilitates access to laboratory testing, including laboratory ordering, requisition delivery, and results delivery. Laboratory orders are coordinated through participating licensed healthcare professionals and laboratory-ordering partners.
Laboratory testing and telehealth treatment are separate services. Unless expressly included in the service purchased, purchasing a laboratory test does not include a telehealth treatment appointment, ongoing medication management, or a prescription. The clinician responsible for a laboratory order may be different from the clinician providing telehealth treatment.
Sara Snarr is a nurse practitioner who provides patient care through telehealth appointments offered through Synergistic Labs. Her role includes patient consultations, review of relevant medical history and laboratory results, and evaluation of appropriate treatment options.
Treatment recommendations are based on each patient’s individual evaluation and clinical needs. Prescription medications are prescribed only when the treating provider determines that they are medically appropriate and applicable requirements are met. Scheduling an appointment or purchasing a service does not guarantee a prescription.
Telehealth Availability, Patient Location & Jurisdictional Compliance
Telehealth treatment through Synergistic Labs LLC is currently available to eligible patients physically located in Indiana or Illinois at the time of their appointment. Availability is subject to the treating provider’s professional licensure, prescribing authority, clinical eligibility requirements, and the specific service requested.
Telehealth services may be provided only where the treating healthcare professional holds the licenses and authorizations required for the patient’s location. Services must be delivered in accordance with applicable state and federal requirements governing professional licensure, scope of practice, standards of care, telehealth informed consent, prescribing, and any required collaborative or supervisory arrangements.
Patients must accurately disclose their physical location before each telehealth encounter. The treating provider will verify the patient’s location and obtain any required telehealth consent before providing care. Acceptance of these Terms does not replace a separately required clinical consent.
Patients who travel or relocate must notify their provider before receiving further telehealth services. An appointment may need to be rescheduled, or care referred to another provider, when the patient is located in a jurisdiction where the treating provider is not authorized to practice. Access to this website or the ability to submit an appointment request does not establish eligibility for treatment.
Laboratory-testing availability is separate from telehealth-treatment availability. Access to laboratory testing in a particular state does not mean prescription treatment or telehealth consultations are available in that state. Laboratory services remain subject to their own state-specific restrictions and ordering requirements.
Telehealth appointments with Sara Snarr are available to eligible patients physically located in Indiana or Illinois at the time of their appointment, subject to applicable licensing requirements and service availability.
This availability applies to telehealth treatment with this provider. Laboratory-testing availability is separate and may differ by service and location.

Clinical Oversight and Treatment Decisions
Sara Snarr provides clinical evaluation and oversight for patients receiving telehealth treatment under her care. This includes reviewing relevant medical history, symptoms, current medications, and laboratory results; evaluating treatment suitability; and determining appropriate treatment and follow-up recommendations.
Clinical decisions are made by the treating healthcare professional using professional judgment, within the scope of the provider’s licensure and prescribing authority, and in accordance with applicable requirements. Marketing affiliates and nonclinical customer-support personnel do not determine treatment eligibility, select medications, or authorize prescriptions.
Prescription medications are prescribed only after an appropriate clinical evaluation and only when the treating provider determines that treatment is medically appropriate. Completing an intake form, scheduling an appointment, purchasing a service, or expressing a preference for a medication does not guarantee a prescription or a particular treatment.
The provider may recommend additional testing, follow-up appointments, an in-person examination, or referral to another healthcare professional before initiating or continuing treatment. Treatment outcomes vary, and no specific result is guaranteed.
Telehealth Availability and Patient Location
Telehealth treatment with Sara Snarr is available to eligible patients physically located in Indiana or Illinois at the time of the appointment, subject to applicable licensing requirements, clinical eligibility, and service availability.
Patients must accurately disclose their physical location for each telehealth encounter and notify the provider of any change in location that may affect treatment eligibility. Availability of laboratory testing is separate from availability of telehealth treatment and may differ by state and service.
Any required telehealth informed consent will be obtained as part of the clinical intake process. Acceptance of these Terms does not replace a separately required clinical consent.
Website Information and Emergency Care
General information on this website, including treatment descriptions, educational materials, and promotional content, is not an individualized diagnosis or treatment recommendation. A treatment described on the website may not be appropriate for every patient.
Telehealth services are not intended for emergencies and may not be appropriate for every medical condition. For a medical emergency, call 911 or seek care at the nearest emergency department. Do not use website contact forms or routine support messages to request emergency care.
2. SERVICES: By agreeing to the terms of service I understand that I have elected on my own volition to order a lab test and am informed that the services are provided by medical physicians and do not include prescription of legend drugs, surgery, or other conventional allopathic medical treatments. I understand that all lab requisitions are ordered by an Ordering Physician/MD. I understand that the lab results received are not a medical recommendation, and not a diagnosis. I will not hold Synergistic Labs or any staff liable for undiagnosed conditions as a result of my inaction to seek primary allopathic care and routine screenings for disease.
3. NO GUARANTEE: I have been informed that the testing made available to me is not intended to diagnose disease from an allopathic model of medicine. All tests are made available to customers within the United States, and by agreeing to these terms I acknowledge that is my country of residence. I understand that payment(s) for products and services are not conditional on my response to care. Please refer to our financial policy regarding refunds.
4. ACKNOWLEDGEMENT/RISKS: I will not hold Synergistic Labs, Darin Fox, or any affiliate or contractor of Synergistic Labs liable for any experience/interaction at a blood draw facility or throughout the testing process. You acknowledge that lab results are can be inaccurate and that Synergistic Labs or its staff will not be held liable. By agreeing to these terms you acknowledge and agree that you will not hold Synergistic Labs, Synergistic Labs Staff, or any affiliate/contractor of Synergistic Labs liable for website glitches processing orders or any experience at a blood draw facility or during the testing process.
5. REFUND POLICY: Synergistic Labs will issue refunds on a case-by-case basis and support can be reached at info@synergisticlabs.net. Refunds will not be issued after testing has already been completed. Refunds will not be issued after a collection has already occurred. Refunds will not be issued after a test kit has been shipped.
6. QUESTIONS AND ANSWERS: By agreeing to this form, you are stating that you have read and fully understand and agree to the terms outlined in this form. You understand that you should not sign this form if all items, including all questions, have not been explained or answered to my satisfaction or if you do not understand any of the terms or words contained in this consent form.
DO NOT SIGN UNLESS YOU HAVE READ AND FULLY UNDERSTAND THIS FORM. I have read and fully understand this consent. All items have been explained, I have had sufficient time to evaluate the information, and my questions have been answered. Knowing the alternatives and risks, I consent to the services.
