Terms of service

1. SERVICES: By agreeing to the terms of service I understand that I have elected on my own volition to order a lab test and am informed that the services are provided by medical physicians and do not include prescription of legend drugs, surgery, or other conventional allopathic medical treatments. I understand that all lab requisitions are ordered by an Ordering Physician/MD. I understand that the lab results received are not a medical recommendation, and not a diagnosis. I will not hold Synergistic Labs or any staff liable for undiagnosed conditions as a result of my inaction to seek primary allopathic care and routine screenings for disease.

2. NO GUARANTEE: I have been informed that the testing made available to me is not intended to diagnose disease from an allopathic model of medicine. All tests are made available to customers within the United States, and by agreeing to these terms I acknowledge that is my country of residence. I understand that payment(s) for products and services are not conditional on my response to care. Please refer to our financial policy regarding refunds.

3. ACKNOWLEDGEMENT/RISKS: I will not hold Synergistic Labs, Darin Fox, or any affiliate or contractor of Synergistic Labs liable for any experience/interaction at a blood draw facility or throughout the testing process. You acknowledge that lab results are can be inaccurate and that Synergistic Labs or its staff will not be held liable. By agreeing to these terms you acknowledge and agree that you will not hold Synergistic Labs, Synergistic Labs Staff, or any affiliate/contractor of Synergistic Labs liable for website glitches processing orders or any experience at a blood draw facility or during the testing process.

4. REFUND POLICY: Synergistic Labs will issue refunds on a case-by-case basis and support can be reached at info@synergisticlabs.net. Refunds will not be issued after testing has already been completed. Refunds will not be issued after a collection has already occurred. Refunds will not be issued after a test kit has been shipped.

5. QUESTIONS AND ANSWERS: By agreeing to this form, you are stating that you have read and fully understand and agree to the terms outlined in this form. You understand that you should not sign this form if all items, including all questions, have not been explained or answered to my satisfaction or if you do not understand any of the terms or words contained in this consent form.
