Laboratory purchases
Laboratory-testing purchases through Synergistic Labs are final and non-refundable. This includes laboratory orders and at-home testing kits. Please review the test, laboratory, collection requirements, and availability of a suitable testing location before completing your purchase.
Laboratory selection and exchanges
Once a requisition has been generated, your lab order cannot be exchanged, transferred, or reissued through a different laboratory. This includes switching between Quest Diagnostics and LabCorp.
An issued requisition may still be presented for testing. Issuing another requisition through a different laboratory could result in two usable lab orders. For that reason, we cannot accommodate laboratory exchange requests after a requisition has been generated.
Incorrect orders and missing items
Review your lab requisition when you receive it. Contact us promptly if it is missing a marker included in the test you purchased or if you received an incorrect order. We will evaluate the issue and work to correct an error.
The laboratory exchange restriction does not prevent correction of an error made by Synergistic Labs when issuing an order. Selecting the wrong laboratory at checkout or later preferring a different laboratory is not an error made by Synergistic Labs.
If an item or medication is missing from an existing order, including a hormone replacement therapy order, contact us so we can review and address the issue. Include your order number and a description of the problem. Do not send unnecessary health information in an initial support message.
Expired requisitions
Requisitions are valid for six months. Refunds cannot be issued for expired requisitions, except where required by applicable law.
Sale items and gift cards
We do not accept returns of sale items or gift cards, except where required by applicable law.
Medication services and related consultations
All payments for medication services are final and non-refundable, except where a refund is required by applicable law. This includes consultation and clinical evaluation fees associated with medication services, medication charges, program or membership fees, and related dispensing and shipping charges, as applicable and disclosed before payment.
The provider group for our medication services through Architek is Architek Health Provider Network, P.C., and its affiliated providers, with lead physician Dr. Alex Jabourian. The dispensing pharmacy is Perfect Rx Pharmacy, 5810 Long Prairie Rd, Flower Mound, TX 75028.
A provider's clinical decision is independent of your payment or preferred treatment. Payment does not guarantee eligibility, treatment, a prescription, a specific medication, or a particular result. Fees for a completed clinical consultation or evaluation associated with medication services are non-refundable even if the provider determines that medication or the requested treatment is not appropriate, except where a refund is required by applicable law.
Applicable charges, this non-refundable policy, any recurring billing, and cancellation requirements are disclosed before payment or enrollment. Cancellation takes effect according to the cancellation terms accepted for the service and applicable law. Canceling a medication service does not make amounts already charged refundable, except where a refund is required by applicable law. This policy does not authorize continued billing after cancellation becomes effective.
This non-refundable policy applies to purchases made under the terms presented to and accepted by you. Orders already placed remain subject to the payment, cancellation, and refund terms accepted for those orders. The laboratory rules above do not establish the payment or cancellation terms for medication services.
Contact us with questions about an existing order, an incorrect charge, or a missing item so we can investigate and address the issue. This policy does not limit any remedy required by applicable law. Do not send medication to Synergistic Labs or a pharmacy without first receiving instructions from the responsible organization.
