About this consent

This page explains telehealth and the consent requested when you receive a telehealth service. Your treating practice is responsible for obtaining and documenting the consent applicable to your visit during clinical intake. Merely reading this page, using the website, purchasing a laboratory test, or accepting website terms does not record your consent to telehealth.

You may review this information before an appointment and ask the treating practice questions before deciding whether to consent. Additional documents may address a particular treatment, medication, practice, or state requirement.

Who provides care

The healthcare professional identified in your appointment or clinical intake is responsible for your evaluation and care. Synergistic Labs LLC facilitates website access and administrative services. Marketing affiliates and customer-support personnel do not make treatment decisions or authorize prescriptions.

For existing patients receiving care from Sara Snarr, NP, clinical evaluation and follow-up remain with her. Encounters under that arrangement are limited to eligible patients physically located in Indiana or Illinois, subject to licensure and clinical eligibility. Contact the treating practice or Synergistic Labs to confirm current appointment or enrollment availability.

Medication services through Architek are being prepared for launch. For that planned service, the provider group is Architek Health Provider Network, P.C., and its affiliated providers, with lead physician Dr. Alex Jabourian. The dispensing pharmacy is Perfect Rx Pharmacy, 5810 Long Prairie Rd, Flower Mound, TX 75028. The provider identified during intake will determine whether the requested service is available and appropriate for you. This planned program does not replace the provider responsible for an existing laboratory order or clinical relationship.

What telehealth involves

Telehealth uses electronic communication to support healthcare when the patient and professional are in different locations. Depending on the service and applicable requirements, it may include video, audio, secure messaging, online forms, and review of medical history, photographs, or laboratory records. The treating provider determines which form of evaluation is appropriate.

The provider may review your symptoms, medical history, medications, allergies, and relevant results; ask additional questions; and determine whether further testing, an in-person examination, treatment, or referral is needed. Telehealth can improve access to an evaluation, but it does not guarantee that care can be completed remotely.

Limitations and risks

  • A remote evaluation may not provide information available during an in-person examination. Incomplete or inaccurate information can affect a clinical decision.
  • Equipment, connection, audio, video, or messaging problems may interrupt or delay an evaluation and require another contact or form of care.
  • Electronic communications involve privacy and security risks, including access by an unauthorized person or by someone using your account or device.
  • A provider may determine that telehealth is unsuitable, that more information is needed, or that treatment should not be prescribed or continued.

Discuss concerns with the treating practice. You may seek an in-person evaluation or care from another appropriately qualified professional. No specific outcome, medication, or prescription is guaranteed.

Your responsibilities

Provide accurate identity, contact, location, and medical information. Tell the provider your physical location at each encounter and notify the practice of travel, relocation, new symptoms, medication changes, or other information relevant to care. Services depend on the provider's authorization to practice where you are located.

Use a private setting and a device and account you control when possible. Tell the provider if another person is present or if you need help communicating. Follow the practice's instructions if a connection fails and use its designated clinical contact method for follow-up. Routine messages should not be used for urgent or emergency concerns.

Prescriptions and medication decisions

Prescription treatment requires an appropriate clinical evaluation and a prescription from an authorized healthcare professional. Consent to telehealth does not establish eligibility for medication or require the provider to prescribe a requested product. Product-specific risks, alternatives, instructions, and any required treatment consent should be discussed with the provider or pharmacist.

Where a treatment involves a compounded medication, it is not FDA approved, and the FDA does not review compounded medications for safety, effectiveness, or quality. This general telehealth consent is not a substitute for the counseling and consent appropriate to a particular treatment.

Privacy, records, and communication choices

Information exchanged for your care becomes part of the records maintained by the responsible organizations and is subject to applicable privacy law and notices. Review the treating practice's privacy notice and our Privacy Policy. You may ask about access to records and confidential communication options.

This consent does not authorize unrelated marketing, the sale of medical records, or recording of a visit without the notice and consent required by applicable law. If a particular communication method requires a separate risk disclosure or authorization, the responsible practice must address it with you. Agreeing to telehealth does not waive applicable privacy rights.

Choosing or withdrawing consent

Your decision is voluntary. You may ask questions, decline telehealth, or withdraw consent for future telehealth care by notifying your treating practice. The practice can discuss available alternatives; further care may require an in-person visit or referral. Withdrawal does not undo services already provided, actions already taken in reliance on consent, or required record retention.

Charges and cancellation rules are governed by the terms disclosed for your service and applicable law. This consent does not establish a consultation fee, promise a free visit, authorize recurring billing, or create a new refund arrangement.

Consent acknowledgment during clinical intake

The treating practice will present the acknowledgment applicable to your service. It may include the following statements:

I have read the telehealth information presented to me and have had the opportunity to ask questions before consenting. I understand the nature, limitations, risks, and available alternatives to telehealth. I understand that clinical eligibility and treatment decisions belong to the treating healthcare professional and that a prescription or particular outcome is not guaranteed. I understand that I can decline or withdraw consent for future telehealth care. I voluntarily consent to evaluation and care through telehealth by the treating professional identified in my clinical intake, subject to the service-specific information and any additional consent presented by that practice.

Emergency care and contact

For an emergency, call 911 or go to the nearest emergency department. Do not use the website, routine email, or a telehealth intake to seek emergency care.
