Laboratory services
Synergistic Labs facilitates laboratory ordering, requisition delivery, and results delivery. Laboratory orders are coordinated through participating licensed healthcare professionals and laboratory-ordering partners. The laboratory selected for your order performs the testing. Unless expressly included in the service purchased, a laboratory order does not include a treatment appointment, ongoing medication management, or a prescription.
The provider associated with a laboratory order may be different from a clinician who separately provides treatment. Contact us if you need help identifying the appropriate organization for an order or record.
Existing telehealth care with Sara Snarr, NP
This information applies to existing patients receiving clinical evaluation and follow-up from Sara Snarr, NP. Encounters under that arrangement are limited to eligible patients physically located in Indiana or Illinois at the time of an appointment, subject to applicable licensure and clinical eligibility. Contact the treating practice or Synergistic Labs to confirm current appointment or enrollment availability.

Synergistic Labs is preparing medication services through Architek. Enrollment availability will be identified on the relevant program page. A medication description is not a prescription, an approval of treatment, or a promise that a service is available in every state.

For this medication program, the treating provider determines clinical eligibility and whether a prescription is appropriate; the pharmacy is responsible for dispensing. These organizations are identified for the upcoming Architek service. The arrangement does not replace a provider responsible for an existing laboratory order or established clinical relationship.
How a medication evaluation works
- When enrollment is available, review the service details, charges, privacy information, and clinical consent presented with the program.
- Complete the provider's intake with accurate identity, location, medical history, medication, and allergy information.
- A licensed provider reviews the information and may request additional details, testing, follow-up, an in-person evaluation, or referral.
- If treatment is appropriate, the provider determines the prescription and follow-up requirements. If treatment is not appropriate, a prescription is not issued. Any charge is governed by the service terms disclosed before enrollment and applicable law.
Prescription required. Completing an intake does not guarantee a prescription. Availability depends on your physical location, the provider's authorization to practice there, and clinical eligibility.
Compounded medications are not FDA approved. The FDA does not review compounded medications for safety, effectiveness, or quality.
