You can use this page to submit a privacy choice or ask about personal information handled by Synergistic Labs LLC. Depending on applicable law, you may have rights to opt out of the sale or sharing of personal information, targeted advertising, or certain other processing. In this context, legal definitions of "sale" or "sharing" may include some disclosures involving website identifiers; they are not limited to exchanging medical records for money.
Our policy does not permit the sale of laboratory results or medical records. This page does not mean we engage in every activity for which a privacy law provides an opt-out right. Our Privacy Policy explains information categories, purposes, recipients, and other available requests.
Submit an opt-out or privacy request
Email info@synergisticlabs.net or call 317-750-9364. Tell us whether you want to opt out of sale or sharing, opt out of targeted advertising, limit an applicable use of sensitive information, withdraw a consent, or make another privacy request.
You do not need an account to submit an opt-out. We may ask for information needed to apply your choice to the relevant information. A sale-or-sharing opt-out does not require an identity-verification process. Do not provide laboratory results, payment-card details, or unnecessary sensitive information in an initial request.
For access, correction, deletion, or a copy of personal information, we may need to verify your identity before responding. An authorized representative may contact us on your behalf; additional steps may be needed to establish the representative's authority, depending on the request and applicable law.
Cookies and browser choices
Where displayed, website cookie-preference controls may be used to manage the options they present. Your browser settings can also be used to manage cookies and similar technologies. Read our Cookie Policy for more information. Blocking essential cookies can affect shopping-cart, account, or checkout functions. Browser settings generally apply to the browser or device on which you make the choice.
Where applicable law requires recognition of an opt-out preference signal, such as Global Privacy Control, that requirement applies. You can use the email address or telephone number above to submit a privacy choice or request help applying a choice to information associated with your account or orders. A browser choice does not replace any separate consent or authorization required for health information.
Promotional messages and requested services
You can unsubscribe from promotional emails using the link in the message or by contacting us. This does not automatically stop necessary order, requisition, result, or service communications. Contact us separately to discuss how those communications are delivered.
A request to stop sale, sharing, or targeted advertising does not itself cancel an order or request deletion of records. Information may still be processed as permitted by law to provide the services you request and meet recordkeeping or other legal obligations.
Clinical and pharmacy records
Requests concerning medical records may need to be directed to the laboratory, treating provider, or pharmacy responsible for them. Existing patients of Sara Snarr, NP, should direct clinical-record questions to the practice responsible for their care.
For the planned medication service through Architek, the provider group is Architek Health Provider Network, P.C., and its affiliated providers, with lead physician Dr. Alex Jabourian. The dispensing pharmacy is Perfect Rx Pharmacy, 5810 Long Prairie Rd, Flower Mound, TX 75028. These disclosures concern the upcoming medication program; they do not reassign responsibility for existing laboratory orders. Contact us if you need help finding the correct organization.
Responses and appeals
We respond according to the timeframes and procedures required by applicable law. If a request cannot be fulfilled, we explain the reason and any available appeal process. To request review of a decision, email us with the subject "Privacy Request Appeal." We will not unlawfully discriminate against you for exercising privacy rights.
