Existing care with Sara Snarr, APN, is limited to eligible patients physically located in Indiana or Illinois, subject to clinical eligibility and service availability. Laboratory testing

Hormone-related laboratory tests remain available as separate testing services. A laboratory purchase does not include a treatment appointment or prescription unless expressly included in the service listing.
